Health/Primary Care Illinois Screening, Brief Intervention, Referral & Treatment Offered in Cook County, Funded by: Federal Funding from the Center for Substance Abuse Treatment through the Illinois Department of Human Services, Division of Alcoholism and Substance Abuse Illinois SBIRT is a partnership with the Illinois Department of Human Services, Division of Alcoholism and Substance Abuse, the Great Lakes Addiction Technology Transfer Center, the Cook County Bureau of Health Service (CCBHS), and TASC. CCBHS staff in Cook County hospitals and clinics conduct screening and assessment of patients to determine substance abuse or dependence. TASC staff coordinate referrals to treatment for dependent patients and provide community case management and follow up to link patients to appropriate treatment services. Health Services Case Management Offered in Chicago, Funded by: AIDS Foundation of Chicago When HIV began its rapid spread through the intravenous drug population, TASC began creating programs designed to prevent the further spread of the virus and to assist HIV-affected drug users in coping with the disease. Today, TASC case management continues to reduce the risk of HIV/AIDS transmission, and substance use intervention programs in primary health care settings offer an opportunity for the early identification and treatment of substance use disorders. TASC provides case management services for clients who may have already had contact with HIV. TASC educates individuals about the transmission of the virus and offers risk assessments, pre- and post-test counseling, and one-on-one consultation with those who request further information. Case managers support clients in accessing appropriate medical care, housing, and support services. |
